As a Medication Safety Officer, you will play a crucial role in developing, implementing, and overseeing medication safety programs across our expansive network, encompassing acute care facilities, ambulatory care centers, home care services, hospice programs, and serving both rural and urban areas.STATUS: Full Time, 40 hours LOCATION: All RRH Finger Lakes sitesDEPARTMENT: PharmacySCHEDULE: Monday-Friday, day shift. Hybrid model if desired. ATTRIBUTESBS Pharmacy or PharmD requiredActive NYS Pharmacy Licensure requiredCompletion of American Society of Health Systems Pharmacy (ASHP) accredited residency program, or 3 years of experience in clinical practice preferredRelevant experience in medication safety, quality improvement, or a similar role within a large integrated health systemStrong understanding of regulatory requirements related to medication safety, adapting to the complexities of diverse care settingsExcellent communication and interpersonal skills to effectively collaborate with a wide range of healthcare professionals across various locationsAnalytical and problem-solving abilities to identify and address medication safety issues in different care environmentsCertification in Medication Safety or Patient Safety is preferredHuman Factors trained preferredJust Culture champion certification preferredRESPONSIBILITIES Develop and maintain medication safety policies and procedures tailored to the unique needs of diverse care settings within RRHCollaborate with healthcare professionals across various departments to identify and address medication safety concerns specific to each care settingInvestigate and analyze medication errors, adverse drug events, and near misses across the entire health system to implement corrective actions and prevent future occurrencesProvide education and training to team members on medication safety best practices, adapting strategies to each care environmentStay informed about the latest developments in medication safety, pharmacovigilance, and regulatory requirements, ensuring system-wide complianceAct as a liaison between departments and locations to facilitate communication and promote a consistent culture of safety throughout RRHParticipate in system-wide quality improvement initiatives, contributing to the development of strategies that enhance patient outcomes across RRHApply knowledge of safety science, care delivery processes, and pharmacology to design safe systems of medication useEDUCATION: BS: Pharmacy (Required)PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.LICENSES/CERTIFICATIONS: LPharm - Licensed Pharmacist - New York State Education Department (NYSED)PAY RANGE: $126,400.00 - $167,500.00 The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
